The Gluteus Maximus is the most coarsely fibered and biggest muscle in the body, forming the bulk of the buttock. The upper fibers laterally rotate the hip joint and may also assist in abduction of that joint. The lower fibers extend and laterally rotate the hip joint (ex. forceful extension, as in running or rising from sitting).Through its insertion into the IT tract, the Gluteus Maximus also helps to stabilize the knee in extension. Some assert (CHEK, Walden) that the gluteus Maximus should be thought of primarily as muscle affecting the knee rather than a muscle of the hip. In any case it is a strong muscle with a lumbo-pelvic pain map.
Consider the Gluteus Maximus in cases of Post-surgical (lumbar) chronic low back pain which don’t respond to conventional treatments. Also in Spondylolisthesis. There are three zones in the muscle which tend to develop trigger points the upper middle and lower. We have found the upper fibers to be most commonly affected producing the most florid pain symptoms.
